Complete a Pink Card or Report Card Online

Every ypink cardear at The Annual Family Cafe, we offer attendees a chance to submit questions and comment to the State Agencies that serve Floridians with disabilities through our "Pink Card" process.

The Family Cafe shares the questions and comments we receive with with the State Agency or office that has the answer, and they respond directly. We also use their replies to create our annual Questions & Answers Book, which we publish in the fall.

Whether or not you were able to attend The 21st Annual Family Cafe, we welcome your questions and comments about Florida's disability service delivery system. You can complete a pink card online here. We will add your question or comment to those we have already collected, and make sure that you get a response.

Additionally, if you were on hand at this year's event, we are also looking for your feedback on what you liked, and what we can do better. If you didn't get a chance to fill out a Report Card at The 21st Annual Family Cafe, you can complete a Report Card form online. We rely on feedback from attendees to help us improve. We would love to hear how you think we're doing!
